CDMAT APPLICATION REQUIREMENTS
- 1. GMAIL ACCOUNT (Use existing or create a new one)
- 2. 2" X 2" PHOTO ON WHITE BACKGROUND (pictured or scanned copy in JPEG format; not selfie photo)
- 3. ELECTRONIC SIGNATURE (picture or scanned copy in PNG Format)
- 4. GRADE RECORDS (scanned copy; front and back; merged in one PDF file)
- - For graduated or graduating SHS student, GRADE-12 REPORT CARD (at least with grades in the 1st semester)
- - For Transferees*, "Transcript of Records" (TOR) or Certificate of Grades (COG)

POLICIES
- Applicants with incomplete and incorrectly submitted requirements shall automatically be rejected.
- Regularly check the inbox and / or spam folder of your registered Gmail account to us to check your application status.
- Failure to attend (no show) on your assigned test schedule would automatically tagged you as failed in CDMAT.
- Checking of application, validation of requirements, and evalutation of grades may usually take at least seven(7)
working days to complete upon successful submission.
- We shall we accomodating 1,600 approved applicants to take the CDMAT per examination date.
- Refrain from submitting an application more than once.
- Please make sure that you have access to your active GMAIL account.
- Prepare a scanned copy of your signature with '.png' file format.
- Prepare a scanned copy of your G-12 card with '.pdf' file format..
- Prepare a scanned copy of your passport size photo with '.jpg'/'.jpeg' file format.
- Always check for typographical errors before submitting your application.
- Please clear your browser's cache to prevent or resolve performance problems.
- Please patiently wait for the system-generated confirmation of your application.

- If you did not received the application confirmation for more than 24 hours,
email us at support.admission@cdm.edu.ph with attached proof of the successful submission and wait for further instructions.
- Any valid or government-issued identification (ID) card.
- COVID-19 vaccination certificate or vaccination card.
- Black or blue ballpen.
- Unsubmitted pre-enrollment requirements
(if there are any, e.g., SHS diploma, Grade-11, and Grade-12 class card, good moral certificate, etc.)
- Two (2) pieces 1” x 1” ID picture with white background.
Note: Write your full name at the back of your 1” x 1” ID picture.
- Extra money to purchase school uniform
- Come on time for the scheduled date. Do not be late
- Wear decent casual attire.
- Bring additional extra money to buy food / snacks and drinks, and other necessities.

- 1 Original Copy of DRUG TEST RESULT (from Ospital ng Muntinlupa or any Accredited Private or Public Hospitals and / or Clinics)
- 1 Original Copy of MEDICAL CLEARANCE(from Ospital ng Muntinlupa or any Accredited Private or Public Hospitals and / or Clinics)
- - Attached with Laboratory Results of the following: Chest X-Ray, Urinalysis (Urine), Hematology (CBC and Blood Type), and Physical Examination
- 1 Original Copy of PSA BIRTH CERTIFICATE
- 1 Original Copy of FORM 138 (CLASS CARD)
- 1 Certified True Copy of SENIOR HIGH SCHOOL DIPLOMA
- 1 Original Copy of CERTIFICATE OF GOOD MORAL CHARACTER (from the School Graduated)
- 1 Photocopy (Front and Back) of MUNTINLUPA CARE CARD for Muntinlupa Residents ONLY
- Come on time for the scheduled date. Do not be late

Admission Fees
- 1. CDMAT Fee: PHP 300.00
- 2. School Uniform:
- Php 2,015.00 for 1 set of complete uniform and 1 set of P.E. uniform (for small to large sizes)
- Php 2,197.00 for 1 set of complete uniform and 1 set of P.E. uniform (for XL to 5XL sizes)
